Job Summary
A healthcare provider is seeking a Patient Care Coordinator (Registered Nurse) to manage client care processes and ensure seamless coordination between patients, caregivers, and the care team. This role is ideal for a nurse with strong organizational and patient management skills.
Key Responsibilities
- Conduct comprehensive client assessments and evaluate care needs
- Manage client intake, onboarding, and care planning with the nursing team
- Assign and schedule caregivers in line with regulatory requirements
- Oversee daily case coordination and monitor service delivery
- Support hospital-to-home care transitions for patients
- Ensure compliance with healthcare standards and policies
Qualifications
- Must be a Registered Nurse (RN)
- Relevant clinical experience is required
- Experience in home care is an added advantage
- Strong organizational and coordination skills
- Excellent communication and interpersonal abilities
- Ability to manage multiple cases effectively
